Technical Assistance:
Technical Assistance was discussed with the provider regarding the timing of emergency evacuation drills.
Comments:
An unannounced monitoring inspection was conducted on 7/12/22 from 10:10am to 11:25am with the provider. There were six children in care, ranging in age from two-years to 13-years-old, for a total of seven points. Children were observed watching a movie and having free play, then playing educational letter games and practicing sight words with flashcards. Five child records, the provider record, and one assistant record were reviewed. The provider and assistant have current certification in CPR and First Aid. The emergency drill log, attendance, and menu were also reviewed. The provider reports that there is no medication for children on site and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. Based on review of five child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance for two children. Evidence: The records for Child A and Child B, with a start date of 6/16/21, did not contain documentation of a physical.
Plan of Correction:
I did have copies of their physicals when they enrolled but they could not be located on the day of the inspection. I will request them again from the parent.

Based on review of five child records, the provider did not obtain all required documentation for child records. Evidence: The record for Child C did not contain the name, address, and telephone number of a second designated person to contact in case of emergency if the parent cannot be reached.
