Areas Reviewed:
8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted on 7/12/22 from 10:10am to 11:25am in conjunction with a monitoring inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The provider record and one assistant record were reviewed for documentation of the mandatory Virginia Preservice Training, the Health and Safety Update Training, and current certification in First Aid and CPR. There are children currently enrolled with diagnosed food allergies and their records were reviewed for allergy care plans. The last lockdown drill was documented on 5/10/22. If you have questions regarding this inspection, please contact the Licensing Inspector, Laura Brindle, at laura.brindle@doe.virginia.gov or 540-905-2062.

Violations:
Standard #: 8VAC20-790-180-B
Description: Based on review of documentation and interview, the provider did not maintain a written hard copy record of daily attendance that documented the arrival and departure of each child in care as it occurred. Evidence: On 7/12/22 at approximately 10:10 am there were six children present in the family day home. The attendance record contained documentation of eight children present. The provider reported that two children had left for Summer School and would return in a couple of hours.

Plan of Correction: Moving forward I will be sure to update the attendance if children leave and will return.

Standard #: 8VAC20-790-190-B-12
Description: Based on review of five child records, the provider did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child B contained documentation that the child is allergic to nuts. The record did not include an allergy care plan from the physician.

Plan of Correction: I will request this from the parent right away.
