An unannounced subsidy complaint investigation was conducted on 7/12/22 in response to a complaint received in the licensing office regarding an allegation that the provider was not operating within the terms of the vendor agreement. Based on interviews conducted, there was insufficient evidence to substantiate the allegation and it was determined to be not valid.
