Comments:
An unannounced renewal inspection was conducted on November 9, 2023, from 9:00am-10:00am. At the time of entrance, ( 8) children ( 21 ) points were in care with ( 2 ) caregivers present. The inspector reviewed 6 children?s records and 2 caregiver/household member records. Children were observed during morning snack time, engaged in blocks and balls, diapering and handwashing was observed. No medications were on site. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Repeat violation and Systemic Deficiency: Based on a review of children records, children records did not contain documentation of the Information for Parents form and Authorization for emergency care. Evidence: The following children records were missing the Information for Parents form: Child #1, first day of attendance 3/8/2023 (Repeat) Child #2, first day of attendance 9/20/2022 Child #3, first day of attendance 12/19/2022 Child #4, first day of attendance 1/24/2022 The following child record was missing the authorization for emergency care: Child #3, first day of attendance 12/19/2022
