Technical Assistance:
The applicant must send evidence to the licensing inspector that the background checks have been requested no later than 10 days of this notification dated 6/30/2023. The applicant must also then forward the results to the licensing inspector upon receipt.
Comments:
A monitoring inspection was initiated and concluded on 6/28/2023. There were 2 children present with the provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 Provider / household member records were reviewed. The inspection started at 11:45 am and concluded at 1:55 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of records and interview, the Provider did not ensure that All employees / Caregivers undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter. Evidence: Two Provider / Household Member records were reviewed. The records for Provider #1 had the most recent results of a criminal record check which was dated 5/12/2018, more than 5 years ago. The records for Household Member #2 had the most recent results of a criminal record check which was dated 5/15/2018, more than 5 years ago. The provider confirmed that updated criminal record searches had not yet been requested.
Plan of Correction:
The provider will complete requests for the Criminal Record Searches for Provider #1 and Household #2 within 10 days. A copy of the requests will be sent to the inspector and the results will be forwarded upon receipt.

Description:
Based on a review of records and interview, the provider did not ensure that a complete record was kept for each enrolled child. Evidence: Five children's records were reviewed. The records for child #2, child #2, and child #3 did not contain the results of a health examination. None of the five records contained the of the parent's hospitalization plan and number or medical assistance plan, if applicable.
Plan of Correction:
The provider will ask the parents of child #2, child #3 and child #5 to obtain a physical examination for their children and provide a copy of the results to the provider within 2 weeks. All parents will be asked to provide their hospitalization plan and number or medical assistance program or indicate "not applicable" on the enrollment form.
