Technical Assistance:
Discussed checking smoke detector to see if battery is low.
Discussed keeping folding cots inaccessible or out of the daycare area when they are not in use to ensure a child does not get pinched if they attempt to open or re-fold them.

Comments:
An unannounced monitoring inspection was conducted on4/22/2024 from 10:30am- 2:15pm. At the time of entrance, 4 children were in care with the provider. The sample size consisted of 4 children?s records and 4 Provider / assistant/ Household Member records. Children were observed engaged in an adult led learning activity, eating lunch which consisted of canned spaghetti o's with tomato sauce and meat balls, pineapple slices and milk, and settling on cots and in mesh sided cribs for rest time. Medication administration was not observed and the provider states that no medication is administered to children in care. The provider states that there are no enrolled children with food allergies or sensitivities. Children are occasionally transported in a vehicle while in care and the vehicle is equipped with car seats and child restraints, is covered with vehicle insurance and has current state inspections.There have been no accidents or injuries requiring first aid or treatment during the current registration period. Children play out doors in the back yard or take walks to the neighborhood park. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Repeat violations were found and are identified within this report.

Violations:
Standard #: 8VAC20-770-40-D-5
Description: Based on a review of records and interview, the provider did not ensure that within 30 days of a person in the home becoming 14 years old, a child protective services central registry check is obtained.
Evidence:

Plan of Correction: The Provider stated that she would request a search of the central registry for household member #3 this week and forward the results to the inspector upon receipt.
The results of the Central Registry Search were obtained and sent to the inspector on 5/7/2024.

Standard #: 8VAC20-850-110-C
Description: Based on observation and interview, the provider did not meet the criteria on the health and safety checklist at the time of the monitoring inspection.
The provider did not take adequate precautions to prevent accidents and injuries to children in care.
Evidence:
1. A plastic swing set located in the backyard where the provider confirmed children sometimes play while in care was unstable and easily rocked back and forth when the inspector lightly pushed it.
2. There was a rotted wooden entry pad located in front of the shed located in the back yard. The wood was splintered and shredded easily when touched.
3. A 5 gallon gas can which was open and partially filled with liquid and a 1 gallon gas can which was closed and partially filled with liquid were sitting at ground level in the backyard.
4. Two square rusted metal propane burners with sharp edges were sitting at ground level in the backyard.
5. A metal vertical smoker approximately 3 feet tall which was missing one of the 4 legs had with material stuffed under the corner without a leg was located at ground level in the backyard.
6. A gallon sized open bucket containing approximately 6 inches of water was sitting at ground level in the backyard.
7. There was a rusty screw driver sitting on the surface of the back yard deck which leads from the sliding glass doors in the main childcare area to the outdoor play area used by children in care.
8. There were 6 rusty screws lying on the back yard deck rail at a height of approximately 4 feet.
9. The hot water at the handwashing sink in the bathroom that children utilize while in care measured 142 degrees Fahrenheit with a digital thermometer at the time of the inspection (REPEAT VIOLATION).
10. The provider confirmed that the children in care had recently played in the backyard and that the aforementioned items which had been placed outside in preparation for trash pickup had been present at that time.

Plan of Correction: The provider stated that these items were a result of cleaning out a shed and that they were supposed to have gone to the curb for trash pickup.
She stated that she will have all of the items that were cited removed and/or repaired by Monday 4/29/2024. She stated that she will take the children on short neighborhood walks in lieu of playing in the yard and will not use this space for the children's outdoor play until the corrections have been made.
