Technical Assistance:
Background checks are a required attachment to the Application for Voluntary Registry.

If completed background checks are not submitted to licensing office prior to certificate expiration date, violation notice may be re-issued or revised to reflect background check violations and certificate will not be issued until background checks are received.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A Renewal inspection was initiated on 5/13/2020 and concluded on 5/13/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff/ Household Member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-180-120-C
Description: Based on a review of documentation and interview, the provider did not ensure that all criteria on the VR Health and Safety Checklist are met.
- The provider did not keep a copy of the physical examination results obtained on each child before or within 30 days after enrollment for each child.
- The provider did not have proof of adequate immunization received on the date of admission (or prior to admission) for each child

Evidence:
Records for 2 children were sent to the inspector for review.
The records for child #1 and child #2 did not contain immunization records.
The record for child #1 did not contain the results of a physical examination.
The provider stated that she did not have the missing requested documentation for child #1 and child #2.

Plan of Correction: The provider stated that she would request that the parent for child #1 and child #2 submit this documentation within 2 weeks - or as soon as doctors offices re-open for non-emergency care after pandemic closures.
