Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 8/18/2020 and concluded on 8/20/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 provider / household member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on a review of records and interview, the provider did not meet all of the criteria on the Health and Safety checklist at the time of the inspection. - Keep a copy of the physical examination results obtained on each child before or within 30 days after enrollment.
- Have proof of adequate immunization received on the date of admission (or prior to admission) for each child and at the end of 90 days exclude any child who is not adequately immunized.
Evidence: The records for 2 enrolled children were reviewed. When documentation for child #1 and child #2 was submitted for review during the inspection, the documentation for child #1 did not include an immunization record or physical. The documentation for child #2 did not include an immunization record. Both children were enrolled for care on 4/13/2020. The provider confirmed that she did not have this documentation.
Plan of Correction:
The provider stated that she would contact the parent this week to request the missing documentation.
