Comments:
A self report was received on 10/05/2023 regarding mandated reporting. A staff interview was conducted and documentation was reviewed. A violation was cited regarding mandatory reporting.

A Monitoring Inspection was also conducted today. A total of 3 children (6pts) with the Provider was present. Observation of the home, programming and backyard was made. The children were observed playing with playdoh and interacting with the provider. The children transitioned to the backyard where they took turns riding the tricycles and playing on the playground equipment. A total of 4 children's records, the provider and all household records were reviewed. No medication needs at this time and no identified food allergies. Standard #: 8VAC20-800-860-A
Description: Based on an interview with staff, a staff failed to immediately call the local department of social services or call the toll free number of the Child Abuse and Neglect Hotline (1-800-552-7096/TDD) whenever there is reason to suspect that a child has been or is being subjected to any kind of child abuse or neglect by any person.

Evidence:
Staff #1 was made aware of a suspected child abuse incident and failed to contact the local department of social services or the Child Abuse and Neglect Hotline.

Plan of Correction: The mandated reporting training will be repeated and the certificate of completion will be forwarded to the Licensing Inspector for review. Additional training will be completed regarding identifying risks and signs of child abuse.

Standard #: 8VAC20-800-240-A
Description: Based on observation of the backyard, one area of the outside was not maintained in a safe condition.
Evidence:
1. A brick sidewalk was found with separated bricks. Several lose bricks were laying on the ground presenting a tripping hazard.
2. One child was found attempting to pick up one of the lose bricks.

Plan of Correction: The bricks will be removed from the backyard and placed where children will not have access to them.

Description: Based on observation of the family day home, children over the age of 2 years had access to stairs without protective barriers.
Evidence:
The door leading to the basement level was open allowing children access to the stairs leading to the basement.

Plan of Correction: The door was immediately closed. The door will remain closed at all times as long as children in care are present.
