Technical Assistance:
Technical assistance provided for hazardous substances not only being kept out of reach of children, but also remaining locked. Assistance provided for proof of birth documentation to ensure all pieces of needed information are documented.

Comments:
A monitoring inspection was initiated on May 29, 2024 at approximately 9:30am and concluded on May 29, 2024 at approximately 11:00am. There were a total of 19 children with 7 staff members present. Children were observed engaging in floor activities, interacting with teachers, and beginning circle time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.011-F
Description: Based on review, the center did not ensure that the license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

Evidence:
The current inspection notice was not posted. The inspection notice that was posted during inspection was dated 11/21/2022.

Plan of Correction: Board was relocated to board in Infant Center.

Standard #: 8VAC20-780-70
Description: Based on file review, the center did not ensure documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.

Evidence:
1. File for Staff #1 only contained documentation for one reference check.
2. File for Staff #2 did not include documentation for any reference checks.
3. File for Staff #3 did not include documentation for any reference checks.

Plan of Correction: Requested written documentation per Human Resources.

Description: Based on record review, observation, and interview, the center did not keep a written record of daily attendance that documents the arrival and departure of each child in care as it occurs for each group of children.

Evidence:
One general attendance record is kept where the parents sign their child in and out each day. No attendance records were kept for each group of children.

Plan of Correction: Created forms for each class.

Description: Based on observation, hazardous substances were not kept in a locked place that prevents access by children.

Evidence:
1. Baby wash labeled ?keep out of reach of children? was accessible to children in the Cocomelon room, Sesame Street room, and extracurricular room.
2. Vaseline labeled ?keep out of reach of children? was accessible to children in the extracurricular room.
2. Disinfectant wipes and hand sanitizer labeled ?keep out of reach of children? was accessible to children in the Cocomelon Room.

Plan of Correction: 1. Removed from center.
2. Removed from center.
3. Will keep in locked yellow cabinet.

Description: Based on observation, the center did not ensure children's hands were washed with soap and running water before and after a diaper change.

Evidence:
Staff member in Baby Einstein's room did not wash the child's hands before or after a diaper change.

Plan of Correction: Reminded staff to always wash hands after each change.

Description: Based on observation, the center did not ensure medication was kept in a locked place using a safe locking method that prevents access by children.

Evidence:
A prescription spray medication was in an unlocked cabinet in the Sesame Street classroom.

Plan of Correction: Staff reminded to lock all medicines. Spray sent home.

Description: Based on record review, the center did not ensure that documentation of emergency evacuation, shelter-in-place, and lockdown drills included the required information.

Evidence:
The documentation of drills only included the type of drill, date, and time. The missing required information was as follows:
-The identity of the person conducting the drill.
-The method used for notification of the drill.
-The number of staff participating.
-The number of children participating.
-Any special conditions simulated.
-The time it took to complete the drill.
-Problems encountered, if any.
-For emergency evacuation drills only, weather conditions.

Plan of Correction: Downloaded updated forms. Will use the updated forms henceforth.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center did not ensure that when food is brought from home, the food container is clearly dated and labeled in a way that identifies the owner.

Evidence:
Food brought from home for two children in the refrigerator were not dated.

Plan of Correction: Sent notes to parents. Parents will inspect for labels.

Standard #: 8VAC20-780-570-A
Description: Based on observation, the center did not ensure when a child is placed in an infant seat or high chair, the protective belt was fastened securely.

Evidence:
Two children in the extracurricular room were placed in a highchair table and not secured with the protective belt.

Plan of Correction: Reminded staff to fasten seats.
