Comments:
An unannounced renewal inspection was conducted today between the hours of 9:45am through 11:45am with the provider on site. There were 6 children (19 points) with the provider. The provider was over the points allowed. A sample of 6 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, tv time and being fed by the provider. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-80-A
Description: Based on review of six children's records, the provider did not obtain verification of the child's identity and age of the child within seven business days of the child's first day of attendance.
Evidence:
1. Child #4 began with the provider on 1/22/2019 and did not have documentation that the child's identity and age had been verified.

Plan of Correction: The parent was contacted during the visit and the parent will bring it tomorrow.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the provider failed to ensure that a caregiver does not exceed 16 points by using the point system.
Evidence:
1. The provider had 6 children with 19 points (Child #1 DOB 10/26/2016, Child #2 3/16/2016, Child #3 8/01/2018, Child #4 7/17/2018, Child #5 9/15/2018 and Child #6 4/26/2018) during the inspection today from 9:45am to 11:45am..

Plan of Correction: I will watch and make sure I don't go over points in the future.
