54.1 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/23/2021 and concluded on 04/27/2021. The Provider was contacted by email to initiate the inspection. There were five children with one caregivers for twelve points which is within ratio for supervision. Reviewed a child's record, medication authorization, physical space (using FaceTIme), trainings and emergency preparation. One medication was reported to be on-site the day of the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-111-700-B
Description: Based on provider statement, no written authorization form is available for an emergency medication,.
Evidence:
Child #2 has an emergency medication on-site and no written authorization from the parent or physician has been completed to keep the medication at the home longer than 10 days.

Plan of Correction: Waiting for child to get into Dr, due to COVID -for new meds and testing.

Standard #: 22VAC40-111-710
Description: Based on provider statement and a visual of the medication, the medication is not in the original container.
Evidence:
The medication for Child #2 is not maintained in the original labeled box with identifying information.

Description: Based on provider statement, a medication is present past the expiration date.
Evidence:
A medication for Child #2 expired 7/2020.

Description: Based on virtual inspection and provider statement, no gate is present to prevent children under the age of 2 years access to the stairs.
Evidence:
No hardware installed gate is present to prevent a child two years of age and under access to the stairs. Child #1 is 11 months of age.

Plan of Correction: Working on a solution. No wall on one side so looking into options.

Standard #: 22VAC40-111-540-B
Description: Based on virtual inspection, a prohibited piece of equipment was present.
Evidence:
A children's walker was in the daycare space.

Plan of Correction: Been taken out of the room.
