A Subsidy Health and Safety Inspection (SHSI) was conducted at the Family Day Home (FDH) of Mary Ugarte located in the town of Lovettsville, Virginia. This inspection was conducted on 09/18/2019, between the hours of 8:30 am through 11:30 am to review supplemental health and safety requirements. An Unannounced Monitoring Inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that were not covered in licensing standards were reviewed during this supplemental inspection.
