Comments:
A Subsidy Health and Safety Inspection (SHSI) was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

This SHSI inspection was initiated and concluded on 01/11/2022. There were no children present with 1 caregiver. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined (1) non-compliance with an applicable standard. This Violation was documented on the Violation Notice issued to the home.

If you should have any questions, please call or send an email to charlie.perkins@doe.virginia.gov. Thank you.

Charlie Perkins,
Licensing Inspector
