Technical Assistance:
Discussion of the storage of medication within the bathroom.

Discussion of submitted FDH Renewal Application; Provider will need to submit names and dates of birth for all adult household members (including herself).

Comments:
This renewal inspection was initiated by licensing staff and concluded on 02/01/2024, between the hours of 9:30 am to 11 am. There was 1 preschool child present (2 points), with 1 caregiver supervising. The child was engaged in the following: toy play, learning, worksheets, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. The following were reviewed: 1 child's record, 1 household record, and 1 staff record.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on review, 1 child enrolled did not have all of the required information contained within their child's record.

Evidence -
1. On the date of inspection (02/01/2024), the following information was not available for review for Child #1: 1 of 2 emergency contact information was listed; the policy number for the child's listed medical insurance; and the authorization for emergency medical care was not signed by Child #1's parent.
2. The start date for Child #1 was listed as 01/03/2024.

Plan of Correction: All missing information will be obtained the child's parent and placed on file.

Standard #: 8VAC20-800-180-A
Description: Based on review, 1 staff member and 1 household member did not have a current Report of Tuberculosis Screening form (TB) (in accordance with the requirements in 8VAC20-800-170), every two years.

Evidence -
1. On the date of inspection, both Staff #1 and Household Member #1 did not have current TB documentation on file.
2. The TB documentation on file for both was dated - 01/19/2022.

Plan of Correction: An appointment has been scheduled to update the TB documentation on file.
