The licensing inspector conducted an unannounced inspection on 09/01/2022 in response to a self report that was received in the licensing regarding allegations in the area of supervision. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on interviews, a record review and review of video footage, the center did not ensure the care, protection and guidance of children when staff are supervising them.
Evidence: On 08/26/2022, around or about 5:05pm, After spitting on the floor, Child 1 began running in the classroom. Staff B followed behind him in an attempt to stop the child from running. As Child 1 came around the the carpet, Staff A extended her leg and foot in the child's path. Child 1 tripped over Staff A's foot and fell to the floor. Staff B picked the child up and he briefly lost consciousness in her arms. Staff A stated she extended her foot in the child's path in an attempt to stop him from running.
Plan of Correction:
The Director and Owner counseled Staff A & B about the incident and discussed safer ways to manage behavior in the classroom.
