Comments:
A Subsidy Health and Safety inspection was conducted on 12/18/2019 from 9:30 am until 12:00 pm at Abundant Love Learning Center-Louisa to review supplemental health and safety requirements. At the time of the inspection 23 children were present with five staff. The sample size consisted of five children's records and five staff's records. No children had medication at the center at the time of the inspection. Children and staff were observed during free indoor play, educational activities, hand washing, bathroom break, preparing for lunch, lunch, nap, transitions and behavioral management. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Based on record review and interview, the vendor failed to obtain documentation determining one out of five staff is free of communicable tuberculosis (TB) that was submitted prior to coming into contact with children and completed with in 30 calendar days of the date of employment.
Evidence:
1. Staff 4's start date was 6/19/19. The TB on file is dated 11/9/18. 2. The director verified the 11/9/18 TB is the only documentation they have.
Plan of Correction:
In the future new staff will be required to provide documentation of TB results that have been completed within 30 calendar days of the date of employment. Staff 4 will be advised to provide a current TB screening.
