Comments:
An unannounced monitoring inspection was conducted today from 10:30am-11:30am. There were 22 children directly supervised by 6 staff. The physical plant, 4 staff records, 7 children?s records, , evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group instruction, and playing outside. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, one of seven children's records reviewed did not contain all required information.
Evidence:
Child 6's record did not contain the name address, and phone number of one of the two required emergency contacts.

Plan of Correction: We will request this information from the parent and update the file.

Standard #: 8VAC20-780-270-A
Description: Based on observation, not all areas and equipment was maintained in a safe condition.
Evidence:
1. Two bikes on the playground were missing handlebar grips, exposing the hollow metal tubing, and posing a finger entrapment hazard.
2. A section of the mulch retaining wall was observed to have an approximate 5 inch crack.

Plan of Correction: 1. We will remove the bikes from the playground immediately and return them when replacement parts are able to be installed.
2. We will fill the crack with filler.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Several cleaning products were observed in an unlocked cabinet in the 4's/5's classroom.

Plan of Correction: The cabinet was locked immediately during the inspection.
