Comments:
An unannounced Monitoring Inspection was conducted today from 5:00pm to 5:42pm. There were 2 school-agers directly supervised by 2 staff. The physical plant, 2 staff records, 3 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed leaving care at the time of the inspection. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not maintain children's records with the required information. Evidence: Reviewed 3 children's records and found: 1) Child 1 did not have documentation of parent work numbers. 2) Child 2 and 3 did not have documentation of emergency contact addresses.
