Provided consultation on: 1) standard 22 VAC 40-185-160B: Acceptable forms of documentation of a negative tuberculosis (TB) screening are 1. A clearance statement signed by a physician, the physician's designee or an official of the local health department; 2. The results of a negative tuberculin skin test (TST); or 3. The results of a chest x-ray negative for tuberculosis disease. 2) 22 VAC 40-185-70A: staff records shall contain the name, address and telephone number of a person to be notified in an emergency
Comments:
An announced, focused, monitoring inspection was conducted from 9:30 -11:30 am offsite at the Ron Rosner YMCA central office. Two staff records were reviewed. If you have any questions about this inspection, contact Donna Liberman at 540-359-5244 or Donna.Liberman@dss.virginia.gov.
Based on review of two staff records, the facility did not ensure each staff record contained all required information. Evidence: the records for staff #1 and staff #2 did not have written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position.
Plan of Correction:
Will ensure that all staff records contain all required written information.
