Comments:
An unannounced monitoring inspection was initiated on 2/26/20 from 12:00pm-1:25pm at the Ron Rosner YMCA office in response to a self-report of forbidden action by a staff member. Staff interviews were conducted and one staff file was reviewed. On 3/2/20 from 2:00pm-4:30pm at Cedar Forest Elementary School, the Licensing Inspectors reviewed video recording of the incident and observed the YMCA program. Based on staff interviews and review of video recording, violations were cited in regards to the self-report.

Violations:
Standard #: 22VAC40-185-400-A
Description: Based on staff interviews and review of video footage, staff members did not promote a child's physical, intellectual, emotional, and social well-being and growth, and did not interact with the child to provide needed help, comfort, and support. Evidence: On 2/19/20 at approximately 3:56pm in the gymnasium, Child 1, an 8-year-old, had arrived to the program upset over personal belongings that had become dirty. Child 1 was attempting to clean the belongings and staff asked the child to move several times and then Staff C picked up Child 1 under the armpits, carried the child approximately five feet, and then dropped the child onto the floor. The child became more upset and began yelling at the staff and crying. Staff B reported that Staff C then tried to grab the child's belongings from their hands and said to the child, "Oh so now you're deciding to cry after getting told no by the teachers." Child 1 then ran to the corner of the room, visibly upset and crying. Staff A followed Child 1 and the child disclosed to her what had happened. Child 1 received a written warning for refusing to move and for yelling at and calling the staff names after being picked up and moved.

Plan of Correction: Frequent review of licensing standards and child guidance with all staff. Remind staff that if they become frustrated to allow another counselor to take over while they collect themselves.
