Comments:
An unannounced monitoring inspection was conducted on 7/18/22 from 10:05am to 11:30am with a manager. There were 20 children in care, ranging from 3rd grade to 6th grade, supervised by two staff. The manager reported that the Kindergarten thru 2nd grade children were currently on a field trip at the water park and would soon return so the 3rd thru 6th grade children could go. The children were observed playing pictionary, washing hands, eating lunch, and going to the restroom to change for the field trip. Five child records and two staff records were reviewed. Additional staff records were reviewed recently for another inspection. The Center has eight staff with current certification in CPR and First Aid, as well as trained in Daily Health Observation. The manager reported that there were no medications on site but the center has four staff current in Medication Administration Training (MAT). The attendance was reviewed. Lunch was provided from home or from the school cafeteria. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 8/11/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of five child records, the center did not obtain all of the required information for each child record. Evidence: 1. The records for Child A and Child B did not contain the name, address, and phone number of a second designated person to call in an
emergency if a parent cannot be reached. 2. The record for Child C did not contain the work place and phone number for either parent.

Plan of Correction: Make sure all child files list two emergency contacts and parents work place and phone.

Standard #: 8VAC20-780-80-A
Description: Based on observation, documentation review, and interview with staff, the center did not maintain a written record of daily attendance that documented the arrival and departure of children in care as it occurred. Evidence: At approximately 10:10am on 7/18/22, 20 children were observed to be in care. Staff #1 verbally reported that 18 children were in care. The written attendance contained documentation of 24 children in care. Upon further questioning, Staff #1 checked the attendance document , made a note of who was present and who went on the field trip, and then reported that four of the children had left on the field trip but were not marked, and 20 children were present.

Plan of Correction: There will be a roster created that will list the children on the field trip.
