Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 7:20 - 8:30 am with staff. A monitoring inspection was also conducted (see that report for details.) Three children have diagnosed food allergies and allergy action plans and medications were reviewed. The emergency drill log was reviewed. Based on review of documentation, the facility failed to obtain an allergy care plan for all children with diagnosed food allergies. Evidence: Child #2 and child #3 did not have an allergy action plan that was signed by a parent and physician.

Will ensure that a signed allergy action plan is in the record.

Description:
Based on review of two staff records, the facility failed to have documentation of current certification in CPR for each staff. Evidence: the CPR certification for Staff #2 expired 11/11/2019.

Description:
Based on review of the emergency drill log, and an interview with staff, the facility failed to practice lockdown procedures annually. Evidence: there is no documentation that a lockdown drill was conducted.
Plan of Correction:
Will train staff and have them conduct a lock down drill no later than 3/27/20.
