Technical Assistance:
Technical assistance was provided regarding on-site staff records.
Comments:
An unannounced monitoring inspection was conducted on 3/30/22 from 3:25pm to 4:00pm with program staff. There were 16 children in care, ranging in age from six-years to 11-years-old, supervised by two staff. The children were observed having free play. Two child records and two staff records were reviewed. The Center has two staff with current certification in CPR and First Aid, and one staff trained in Daily Health Observation Training. The staff reported that no medication was on site for children. The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 6/23/21 and the most recent Health Inspection on file was dated 4/9/21. Based on review of documentation and interview with staff, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: At approximately 3:30pm on 3/30/22 the attendance record contained documentation of eight children present. There were 16 children present. Staff reported that the attendance record was from the morning and had not been updated for the afternoon. Dismissal was at approximately 12:30pm.
Plan of Correction:
On April 4th, the site child care manager met with on-site staff to review proper attendance procedures. Procedures reviewed included noting the time the child enters and exits the program on the attendance roster and contacting the school office to verify any absences for afternoon care.

Description:
Based on review of documentation and interview with staff, the program did not implement a monthly practice evacuation drill. Evidence: The drill log for the current school year did not contain documentation of an evacuation drill practiced in October 2021 or November 2021.
Plan of Correction:
We will be sure to practice a monthly evacuation drill moving forward.
