Technical Assistance:
An Allowable Variance was approved for a resident for a criminal conviction. The waiver is available for review at the home.
Comments:
An unannounced renewal study was conducted today. there are 4 children (8 points) with one caregiver. 2 adult and 3 children's records were reviewed. See violation notice form missing item. Children were observed playing, watching a video, washing hands, using the bathroom, eating lunch and preparing for rest time. Rabies vaccination for the dog is current. Emergency evacuation drills, shelter-in-place drills, emergency supplies and first aid supplies are all in compliance. Based on a review of records, one child's record did not contain documentation of what document was viewed for proof of birth verification. Evidence: The child record showed a document was viewed: however, no notice of which form of documentation was viewed. (Child 1)

The parent has been asked to bring the birth certificate in again for documentation.

Description:
Based on direct observation, a small appliance was plugged in while not in use. Evidence: A hair dryer was plugged in while not in use, in the upstairs master bathroom.
Plan of Correction:
It was unplugged during the inspection. I will make sure to unplug the hair dryer daily.

Description:
Based on a provider's statement, there is no fire extinguisher available. Evidence: The provider is unable to locate the fire extinguisher for the home during the inspection.
Plan of Correction:
A new one has been ordered, and I will find the one that is in the house.
