Based on record review, the provider failed to obtain written permission from the parent of each child for the provider to take the child off the premises of the family day home. Evidence: The provider stated that the children go on walks in the neighborhood. Child 1 and Child 2's record did not contain written permission from the parents for walking trips.
Plan of Correction:
I gave the parents the field trip permission form. To obtain signatures to gain consent to carry out the Field Trip throughout the neighborhood.
