Comments:
An unannounced monitoring inspection was conducted today from 11:30am-12:10pm. There were 2 children directly supervised by the provider. The home, 1 caregiver record, 2 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-980-A
Description: Based on record review, the provider failed to obtain written permission from the parent of each child for the provider to take the child off the premises of the family day home.
Evidence:
The provider stated that the children go on walks in the neighborhood. Child 1 and Child 2's record did not contain written permission from the parents for walking trips.

Plan of Correction: I gave the parents the field trip permission form. To obtain signatures to gain consent to carry out the Field Trip throughout the neighborhood.
