Technical Assistance:
We discussed the requirements for staffing during all hours of the program. The risk assessment for a violation was provided.

Also, we talked about sight and supervision requirements. Also, we talked about the importance of accounting for all children's whereabouts outside while a separate program is using the grounds.

Comments:
Thank you for your assistance during the unannounced monitoring inspection conducted from 2:45 PM to 5:55 PM. Today, there were twenty-seven children, ages five through ten years, with two to four staff. I viewed program activities, snack, nutrition, equipment, daily routines, staffing, supervision, interactions with children, injury records, agency reports, emergency supplies and drill records, emergency plan, five records for children, four staff records, staff training, indoor and outdoor areas. The staff were observed providing frequent interactions with the children and responding to individual needs. There was an opportunity to do homework and a staff person was readily available to provide needed assistance. The children enjoyed active play outside.

Violations:
Standard #: 22VAC40-185-350-C
Description: Based on observation of the staff supervising children, and interviews with staff, the staff-to-children ratio applicable to the youngest child in the group and applied to the entire group was not always met. Evidence: Evidence: At 3:29 PM as children arrived for the beginning of the after school program, during the first minute, as many as twenty-one elementary school children were with only one staff person. Staff member 1 was observed watching children at the stage as they obtained a basket for their belongings and were instructed to sit at the steps of the stage. At 3:30 PM, there were twenty-four children with the same staff member 1. The youngest children were age five and the staffing ratio requirement was 1 staff:18 children. At 3:31 PM as the final three children arrived, staff member 2 and 3 arrived to begin work. The staff indicated that one staff member who is usually present for children's arrival arranged to be off work.

Plan of Correction: The director or back-up director will make sure at least two staff are present by school dismissal to meet the required ratio of one staff per every eighteen children. As needed, the site staff will request help from administrative staff of the Boys and Girls Clubs so that a substitute staff person is arranged in time.
