A Subsidy Health and Safety inspection was conducted on November 29, 2022 to review supplemental health and safety requirements. This onsite visit was conducted from 3:30 p.m. through 5:00 p.m. Upon arrival, there were 21 children 3 staff supervising. Four children records and five staff records were reviewed and updated.
A licensing code compliance inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Areas of non-compliance may be found on the violation notice.

Based on record review the center failed to have one staff member complete 16 hours of annual training to include the departments health and safety course update.
Evidence:
1. Five staff records were reviewed and updated. 2. Staff #4 and Staff #5 had not completed the departments annual 3 hour health and safety update. 3. Staff #4's completed the required 10 hours of health and safety training on 01/21/2021. 4. Staff #5 completed the require 10 hours of health and safety training on 06/05/2019. 5. The director confirmed the 3 hour update had not be completed for Staff #4 and Staff #5.
