Comments:
An unannounced monitoring inspection was conducted with center staff on September 26, 2019 to determine compliance with the licensing standards. Staff files were reviewed on October 4, 2019. There were 25 school age children with two staff actively supervising. The children were having snack and playing a group game in the gym. Snack served today: yogurt and bananas. Five children's files were reviewed - please see Supplemental Summary for details. One medication was reviewed and is in compliance. Reminder: an evacuation drill must be done before Tuesday. The last fire inspection was conducted 4/30/2018. The last health inspection was conducted 5/15/2019. Based on review of five children's records, the facility failed to have all required information on file for each child. Evidence: Child 4 did not have documentation of the addresses for two emergency contacts.
