Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program. A monitoring inspection was initiated on 9/24/2021 and concluded on 9/24/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 28 children present, ranging in ages from 5 years to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of two staff records, the facility failed to have documentation that each staff received orientation training by the end of their first day of assuming job responsibilities. Evidence: Staff A (date of hire 8/26/2021) and Staff B (date of hire 8/17/2021) did not have documentation of orientation training until 9/23/2021.
Plan of Correction:
Corrected: Manager provided orientation training to staff again when documentation could not be located.

Description:
Based on review of two staff records, the facility failed to have each employee complete a sworn disclosure statement prior to employment. Evidence: Staff A signed and dated the sworn disclosure statement on 8/2/2021, but did not complete the five required questions on the statement.

Staff A will be asked to complete the sworn disclosure statement.

Description:
Based on review of two staff records, the facility failed to have documentation of a search of the central registry within the first 30 days of employment for each staff. Evidence: there was no documentation of a search of the central registry on file for Staff B (date of hire 8/17/2021).

Manager will check to see if a search of the central registry has been submitted for Staff B.

Description:
Based on review of two staff files, the facility failed to conduct a sex offender registry check and a search of the central registry for any state that an employee has resided in the past five years. Evidence: On on the sworn disclosure statement, Staff B indicated residence in Colorado in the past five years. There was no documentation to support that a sex offender registry check had been conducted prior to hire in Colorado for Staff B. There was no documentation to support that a search of the central registry check had been submitted for Staff B to Colorado in the first 30 days of employment.
Plan of Correction:
The out of state background checks will be conducted and documented.
