Comments:
A monitoring inspection was conducted on 6/01/2022 with center staff and center director. There were 26 children present, ranging in ages from 5 years to 12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were engaged in a variety of activities including manipulatives, games, and making loop bracelets. One staff file was reviewed 6/6/2022. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on staff interviews, the facility failed to have a record available for each child in care. Evidence: a record was requested for Child A, but staff stated it could not be located.

Staff will obtain the information needed for the record prior to the child attending this fall.

Description:
Based on review of the emergency drill log, facility failed to maintain a record of the dates of the monthly practiced evacuation drills for one year. Evidence: there was no documentation of a practiced emergency evacuation drill for March 2022 and April 2022.
Plan of Correction:
Emergency evacuation drills will be practiced and documented monthly.
