Technical Assistance:
One of more background checks were missing or incomplete during the renewal inspection. The provider has been informed that the renewal application would not be processed until the background check results have been reviewed by the licensing inspector.
Comments:
An unannounced renewal inspection was conducted today between 11:00am and 12:05pm. There were 4 children present with the Provider for a total of 9 points within the supervision guidelines. The children were observed playing freely during the inspection. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on interview, the provider has not obtained a central registry background check for a person residing in the home that has become 14 years old. Evidence: A household member turned 14 years old in June 2023. The provider confirmed that central registry background check results have not been received.

I did it but didn't get the results. I'll call them to see where the results are.

Description:
Repeat Violation. Based on observation, meals and snacks brough from home was not clearly labeled to identify the owner. Evidence: Four out of four lunch totes were observed were not labeled with each child's name.
Plan of Correction:
I always label the lunch boxes but the parents take the labels off. I'll tell the parents to leave the labels or I'll do it everyday.

Description:
Based on observation, information contained in children's records was not kept confidential. Evidence: The supplemental page of the most recent inspection was observed posted on the providers wall. The supplemental page clearly states "DO NOT POST".

Description:
Based on record review, children's records did not contain all required information. Evidence: 1. Child #2's record did not contain addresses for both emergency contacts. 2. Child #3's record did not contain the families medical insurance policy number. 3. Child #3's record did not contain the Information for Parents document.

Description:
Based on observation children under two years of age had access to stairs. Evidence: The child care program takes place in the providers lower level and stairs lead to the basement program. A child safety gate was observed leaning against the wall at the bottom of the staircase. Child #4 is 17 months old and can access the staircase.
