Comments:
An unannounced monitoring inspection was conducted today between 1:15pm and 2:05pm. There were 5 children present with the Provider for a total of 10 points within the supervision guidelines. The children were observed playing freely. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Repeat Violation. Based on record review, children's records did not contain all required information. Evidence: Child #5s record does not contain documentation that the family has received the providers insurance declaration.
Plan of Correction:
I thought that I gave the family the form. I have extras. I'll have them sign it today.
