Comments:
An unannounced monitoring inspection was conducted on 10/31/2022 from 12:45pm to 1:50pm. There were 23 School-Agers directly supervised by 8 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside. Hand-washing procedures were observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that physical examinations and immunizations were signed by a physician, their designee or an official of a local health department. Evidence: Reviewed 5 children's records and found that Child 3 did not have documentation of a signed physical examination or immunization record.
