Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A (monitoring inspection was initiated and concluded on 2/12/2021. The provider was contacted by telephone to initiate the inspection. There were 9 children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of records, the provider did not obtain documentation that each enrolled child has been examined by a physician within 30 days of attendance.

Evidence:
Two children's records were reviewed.
The record for child #2 did not contain the results of a physical examination.
The provider was unable to provide documentation of child #2's physical examination results during the inspection. She stated that she thought she had them but was unable to locate them.

Plan of Correction: The provider stated that she would request a copy of results of a physical examination today to be obtained by the end of next week.
