Comments:
A Renewal inspection was initiated and concluded on 3/23/2023. There were 12 children present with 3 providers supervising. The point total was 24. The inspector revied compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4r caregiver / household Member records were reviewed. The inspection started at 12:30 pmand concluded at 2:10 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of records and interview, the licensee did not obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence: Three caregiver records were reviewed. The record for caregiver #3, who began employment on 3/17/2023, did not contain results of a Tuberculosis Screening. The licensee confirmed that she had not received this documentation from caregiver #3 who was working with the children during the inspection.
Plan of Correction:
The licensee will have caregiver #3 obtain a TB Screening and place the results in her record within 30 days.

Description:
Based on a review of records and interview, the licensee did not obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: Three caregiver records were reviewed. The record for caregiver #2, who was working with children during the inspection, contained the most recent results of a Tuberculosis screening which was dated in 2019 and was more than 2 years ago. The licensee confirmed with caregiver #2 that she did had not obtained a more recent screening.
Plan of Correction:
The licensee will have caregiver #2 obtain an updated TB Screening and place the results in her records within 30 days.
