Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 11/6/20 and concluded on 11/10/20. The provider was contacted by email to initiate the inspection. There were 5 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s record submitted by the facility to ensure documentation was complete. The staff records were up to date, so no documentation was needed to review. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Based on file review, 1 out of 2 children's records did not include all required information. Evidence 1. Child #1's record did not include the father's employment address or the policy number for the child's health insurance.
Plan of Correction:
The Provider will ask the parent to complete this.
