Comments:
An unannounced monitoring inspection was conducted today from 12:40 pm-3:10 pm. There were 38 children (infants-5yrs.) directly supervised by 6 staff. The physical plant, staff records, children?s records, medication policies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in naptime, reading books, eating snack. and parent pick up. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Based on review of four staff records, the center did not update criminal history record reports every five years.
Evidence: Staff A's nationwide fingerprint search expired on 7/11/23 and has not been repeated as required,

I will send her today to do fingerprints.

Description:
Based on record review, staff members did not obtain and submit results of a follow up tuberculosis (TB) screening at least every two years as required.
Evidence:
Staff A- last TB screen expired 7/2023 and had not been repeated as required. Staff B- last TB screen expired 11/22/23 and had not been repeated.
Plan of Correction:
I will have then do the TB test.

Description:
Based on observation, staff to child ratios for 2 year olds ( 1 staff to 8 children) was not followed while children were in care.
Evidence:
1. On 4/3/2024 There was 1staff with 9 children when the inspector walked into the classroom. 2. From approximately 2:30 pm- 2:45 pm there was only 1 staff in the classroom as nap was ending. 3. Staff returned to classroom at 2:45 pm after preparing snack.
Plan of Correction:
The doubled the ratios at naptime. The second teacher was trying to help get snack ready since she saw that I was busy with the inspector. The children woke up while she was helping.

Description:
Based on observation, the surface used for diapering was no longer considered nonabsorbent. Evidence: There was a rip in the plastic covering on the diaper changing pad in the infant room.

I have already ordered a new one.
