Comments:
An unannounced monitoring inspection was conductedon April 24, 2024 approximately 111:00 am to 11:30 am. There were 6 children for a total of 18 points directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed transitioning from lunch to indiviudal play and resting. There was an assortment of age appropriate materials for children.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of three household members? records and interview with the provider, the provider did not obtain criminal history record reports for household members within 30 days of their becoming 18 years old.

Based on household records reivew, the provider did not obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening . Evidence: 1. Household Member #3 did not have not have a initial Tuberculosis Screenig prior to coming into contact with enrolled children in the home.
