Technical Assistance:
Provided clarification regarding the requirements and timeframes for child immunizations and updated immunizations for all children under the age of 2 years.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 10 children (28pts) with 2 staff were present. The children were observed playing in the programming space with toys and equipment within reach to enable independent selection. One child was observing sleeping in the crib room. Observation of the crib room, both nap rooms, program rooms, diapering area, outside play space and kitchen was made. The older children were observed transitioning to a learning activity with the provider and the younger children were supervised by the assistant. Hand washing procedures were observed prior to sitting down for lunch. One medication need currently. A total of 5 children's records and all staff records (2) were reviewed. Previous violations from the last inspection were reviewed and corrective actions were verified for all but one repeated issue. This inspection began at approximately 10:30am and ended at 12pm. Standard #: 8VAC20-800-750-A
Description: Based on observation of the diapering area, Topical ointments were not properly labeled.
Evidence:
Two topical ointments were found that were not labeled with a child's name.

Plan of Correction: All topicals will be labeled with the child's name.

Standard #: 8VAC20-800-110-B
Description: Based on review of child records, immunizations were not signed by a licensed physician or physician designee.
Evidence:
Child #1 and #3's immunizations were not signed by a physician or a physician's designee.

Plan of Correction: Any new children enrolled will have immunizations signed by a doctor and on file before they start care.

Standard #: 8VAC20-800-50-A
Description: Based on review of records, one record was not maintained.
Evidence:
A written record for each child each child in attendance was not maintained.

Plan of Correction: Attendance for today was completed. Attendance will be taken as each child arrives everyday.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, one child began care before the provided obtained documentation that the child was adequately immunized.
Evidence:
Child #2 began care on 08/22/2022. Immunizations were signed and dated by the physician on 08/29/2022.

Plan of Correction: I will obtain immunizations signed by a doctor that includes a statement that the child is adequately immunized before they begin care.

Standard #: 8VAC20-800-250-A
Description: Based on observation of the family day home, one hanging item was found within reach of children under the age of 5 years.
Evidence:
Blind cords located in the name room were within reach of children in care.

Plan of Correction: Cords will be wrapped and secured out of reach from all children.

Standard #: 8VAC20-800-430-D
Description: Based on observation of the front porch, the protective barrier at the top of the steps was not constructed to prevent a child from falling through.
Evidence:
The gate located at the top of the steps leading to the side yard did not latch to prevent a child from falling down the steps.

Plan of Correction: A new gate will be installed at the top of the stairs leading to the side yard.
