Two children are enrolled and have flexible / changing schedules of attendance. The enrolled children are 11 years old and 12 years old.
The inspector reviewed compliance in the areas of Provider registration and general procedures, Health and Safety, Access to minor?s records, Background Checks, storage of firearms, and Child abuse and neglect. A total of 2 child records and 1 provider record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-110-C
Description: Based on a review of records and interview, the provider did not meet all criteria on the health and safety checklist at the time of the inspection; "I keep a copy of the physical examination results obtained on each child before or within 30 days after enrollment."

Evidence:
Two enrolled children's records were reviewed during today's inspection.
The record for child #1 and #2 did not contain the results of a physical examination.
The provider confirmed that she did not have documentation of child #1 or child #2's physical examination.
