Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A Renewal inspection was initiated on 8/13/2020 and concluded on 8/24/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-180-120-C
Description: Based on a review of records and interview, the provider did not meet all of the criteria on the health and safety checklist at the time of the inspection.

Evidence:
The documentation provided during the inspection at the inspector's request did not include for child #1 and for Child #2
- the names and phone numbers of one or more persons besides the family who may be contacted in case of an emergency.
- a copy of the physical examination results obtained on each child before or within 30 days after enrollment.
When asked for this documentation during the inspection, the provider stated that she did not have it.

Plan of Correction: The provider states that she will obtain the missing documentation from the children's parent (they are siblings) and maintain it in her records for future review.
