Comments:
An unannounced monitoring inspection was conducted August 22, 2023 between 1:57pm and 3:20pm. There were 5 children present with one (1) caregiver supervising for a total of 10 points within the supervision guidelines. The children were observed sleeping, diapering and toileting procedures, handwashing, playing with age-appropriate toys and materials. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. No medication on sight at the time of the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, the provider's records are not maintained in the home and accessible to the department's representative. Evidence: 1. Attendance records for previous weeks were not accessible. 2. The provider acknowledged she did not maintain attendance records and disposed of them weekly.

Based on record review, it was determined documentation of additional immunizations was not obtained every 6 months for children under the age of 2 years. Evidence: The record for Child #1 (DOB 07/2021) did not have documentation of additional immunizations. Last update was 12/2/2022.
This is a repeat violation.
Plan of Correction:
I will tell the parent to bring me an updated immunization record.
