Technical Assistance:
!. Consultation provided on storage of staff purses, the location of the outlet power strip in the infant room.
2. Per standard 430C, there should be no protrusions on the play equipment. Please cut the bolts on the swings to ensure they don't protrude more than 2 threads and make sure once they are cut they don't present with sharp edges.
3. Consultation provided on translating high school diplomas into English.

Comments:
An unannounced renewal inspection was conducted today with administrative staff from 12:30 pm to 2:15 pm. There were thirty-nine children present with nine staff providing supervision. The schoolagers had returned from playing mini golf and had eaten lunch ( chicken nuggets, macaroni noodles, cucumbers, oranges and milk) and were resting in preparation to go outside onto the playground. All the preschoolers were napping during the walkthrough. Two buses were inspected today as well as five staff and children's files. A sample of 8 injuries/accident reports were reviewed as well. There were medications on site for any of the children.

Violations:
Standard #: 8VAC20-780-430-C
Description: Based on observation, it was determined that the play equipment on the playground presented a hazardous protrusion.
Evidence:
On the top and side of the swing bar were protruding bolts more than two threads. These protrusions present impalement and laceration hazards.

Plan of Correction: We will place the swings off limits until we are able to cut the bolts. After cutting the bolts we will file down the edges so that there are no sharp edges.
