An unannounced monitoring inspection was conducted on February 28 2020 between 11:00am and 12:15pm. There were 7 children in the direct care of the provider and 2 assistants. Children were observed playing with stacking cups, being diapered, washing hands, and eating lunch. An area of non-compliance is listed on the violation notice. Based on record review, 1 of 7 children's records reviewed did not have all of the required information.
Evidence: Child #6 record does not have documentation of 2 emergency contact addresses. The record has listed both contacts and telephone numbers, but is missing the addresses.
