Comments:
An unannounced renewal study was conducted from 1:00 pm - 2:00 pm. There were five children in care with the provider and one assistant for a total of nine points. The children were observed resting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, three household member records, and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-80-A
Description: Based on review of five children's records it was determined that not records contained documentation of children's proof of identity within the first seven days of enrollment. Evidence: The record for child A (start date: 03/31/23) did not contain documentation of proof of identity.

Plan of Correction: The provider requested proof of identity from the parents of child A during the inspection.

Standard #: 8VAC20-800-280
Description: Based on observation it was determined that not all potentially poisonous agents were inaccessible to children. Evidence: A spray bottle labeled bleach and water was observed hanging on the edge of the changing table which was accessible to children.

Plan of Correction: The spray bottle was locked during the inspection.
