Based on record review and interview on 02/07/2020, the provider did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: 1. The record of Child #1 (DOE: 09/09/19) did not contain documentation of an allergy care plan. 2. During interview, the administrator reported a written allergy care plan has not been obtained for Child #1.
Plan of Correction:
Per the Center: "PLAN OF CORRECTION: Prior to the inspection, the director provided the child's father a form to be filled out by the child's physician. DATE TO BE CORRECTED: The director has communicated with the father about the importance of having this documentation at the center. Documentation will be present no later than March 6th or the child will not be able to continue attending before/after school."

Evidence: The record of Staff #4 (DOH: 9/17/19) contained documentation of Virginia Pre-Service training dated 2/6/20, which exceeds 90 days from the date of hire.
Plan of Correction:
Per the Center: "PLAN OF CORRECTION: The director will be more aggressive in ensuring new staff complete the Virginia Pre-Service Training withing 90 days of employment. DATE TO BE CORRECTED: The staff member completed the training on 2/6/20. This was prior to the inspection on 2/7/20."
