Comments:
An unannounced monitoring inspection was conducted on November 19, 2019 with center staff to determine compliance with the licensing standards. There were 46 children present, ranging in age from 4 years to 12 years, with three staff actively supervising. The children were arriving from school, eating snack, and working on homework. Snack served today: rice krispie treats and Capri Sun juice. No files were available on site for review. The facility does not administer medication. One staff present has current certification in first aid and CPR. Based on observation and staff interview, the center failed to keep at the center a separate record for each child enrolled. Evidence: the center owner was not present at the facility on the afternoon of November 19, 2019. The three staff on site did not have access to the children's files because the center owner "takes them back and forth".

Description:
Based on observation and staff interview, the center failed to keep a written record of children in attendance daily. Evidence: the children arrived from school between 3:50 p.m. and 4:00 p.m. At approximately 4:45 p.m., the licensing inspector asked to see the attendance record. Staff counted the children and gave the inspector a head count. There was no written documentation of which children were actually present.

Description:
Based on observation and staff interview, the facility failed to have all children in actual sight and sound supervision of staff. Evidence: Staff 1 took 13 preschool and kindergarten students to the rest room down the hall. Staff 1 returned to the cafeteria for about 5 minutes. Staff 1 then stated she had to go back to get the children from the rest room. There were no center staff present at the rest room for a period of about 5 minutes.
