Comments:
An unannounced, on-site monitoring inspection was conducted on May 20, 2024. The on-site inspection began at 4:00 p.m. and ended at 5:30 p.m. The inspector reviewed compliance in the areas listed above. There were 34 children present and three staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks every five years. The most recent central registry check for Staff 2 was dated 1/1/2019. The most recent national fingerprint check was dated 4/29/2019.

The background checks will be repeated every 5 years.

Description:
A tuberculosis (TB) screening is required every two years from the last screening. Staff 1's screening was dated 1/25/2022, Staff 2's screening was dated 3/13/2022, and Staff 3?s screening was dated 3/30/2022.

Description:
Each center shall maintain and keep at the center a separate record for each child enrolled which contains required information. The center did not have access to the children's physical and immunization records that are housed in the school office. The office was closed during the center's hours of operation on 5/20/2024.

The center will obtain copies of the children's physicals and immunization records if the school office is not open during center's hours of operation.
