Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection initiated on May 18, 2020 and concluded on May 19, 2020. The provider was contacted by telephone to initiate the inspection. There were three children and two staff. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children?s records and two caregiver records submitted by the family day home to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, in two of two children's records reviewed, the provider did not ensure that each child's record contained all required information.
Evidence: The provider confirmed that the following information had not been documented in each child's record: Child #1-policy number of the child's medical insurance Child #2-complete address for one parent's place of employment
Plan of Correction:
Provider will have the parent to put the policy information on the form for Child #1. Provider will have the parent to update the address for Child #2.
